Nightingale Home Healthcare is seeking two full-time Registered Nurses (RNs) to join our growing Stamford-based team. This is a high-impact, territory-based role where you will manage patients across Stamford and surrounding areas with full clinical autonomy and strong organizational support. • Manage a defined patient population in Stamford and nearby areas
• Perform Start of Care (SOC), Recerts, and routine visits
• Complete OASIS assessments and develop Plans of Care
• Monitor patient progress and adjust care accordingly
• Coordinate with therapists, aides, and physicians
• Ensure timely, accurate documentation in Well. Sky
• Deliver high-quality, patient-centered care in the home setting Qualifications: ✅ What We’re Looking For
• Active RN license in Connecticut
• Home Health experience REQUIRED
• OASIS experience strongly preferred
• Strong clinical judgment and independence
• Reliable transportation
