We are currently seeking an experienced: CENTRAL INTAKE SCHEDULER JOB SUMMARY: Coordinate and schedule all admission and routine visits for hospice, home health, and palliative patients, with priority given to urgent and same-day requests.

Maintain proactive and ongoing communication with referral sources, marketing team, field clinicians, intake staff, and patient families to ensure clarity, consistency, and timely follow-up. Accurately input and update all scheduling activities within the EMR to support data integrity and meet regulatory standards. Anticipate, identify, and resolve scheduling conflicts or delays in the admission process to prevent service disruptions.

Collaborate closely with interdisciplinary teams to support smooth transitions of care and ensure appropriate staff assignments. Maintain clear, organized documentation of all scheduled admissions and completed visits for tracking and reporting purposes. Perform additional duties as assigned to support departmental goals and patient care outcomes.

JOB REQUIREMENTS: High school diploma or equivalent required Associate degree or healthcare-related coursework preferred Experience in Microsoft 365, medical scheduling, admissions coordination, or hospice/home health preferred. Knowledge of healthcare terminology and processes. Strong organizational and time management skills, with the ability to prioritize tasks, manage multiple schedules, and meet tight deadlines.
