This role offers the opportunity to make a meaningful impact by coordinating comprehensive care, supporting patients and families through the hospice journey, and collaborating closely with the clinical team to ensure the highest quality of care.
Responsibilities
Coordinates and oversees all aspects of patients' home care. Collaborates with patient care teams to develop and implement care plans. Educates families on patient condition, needs and care. Performs head to toe physical assessments. Checks vital signs and manages medications. Administers and maintains catheters. Complete documentation timely and accurately.
Supervises HHAs. Regularly communicates patient progress to the clinical manager, physician, and care team. Coordinates and oversees all aspects of new patient admissions. Other duties as assigned.
Requirements
Current State license as a Registered Nurse. Current driver’s license. Current CPR Certification. Experience: Two (2) years' experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice preferred. Skills: Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations.
Ability to work independently and make sound decisions. Strong attention to detail and organizational habits. Excellent verbal and written communication skills. Good interpersonal skills. Transportation: Reliable transportation and valid auto liability insurance.
