Home Care Field Coordinator

Overview

We are seeking a dedicated and organized Home Care Field Coordinator to join our team. This role involves coordinating patient intake, providing support during home visits, and assisting nursing staff with various operational tasks. The ideal candidate will be proactive, personable, and capable of managing multiple responsibilities in a dynamic home care environment.

Responsibilities

Conduct in-home intake assessments for new patients, ensuring a welcoming and professional experience. Meet and greet patients and their families to establish rapport and gather necessary information for care planning. Assist nurses with any issues that may arise during home visits, providing support to ensure quality patient care.

Manage EVV (Electronic Visit Verification) timesheets accurately and timely to ensure compliance with documentation requirements. Coordinate scheduling and logistics for home care visits, optimizing efficiency for staff and patient needs. Maintain clear communication with nursing staff, patients, and family members to facilitate seamless care delivery.

Support the onboarding process for new patients, including documentation and initial assessments as needed. Skills Excellent interpersonal and communication skills to interact effectively with patients, families, and healthcare professionals. Strong organizational skills with the ability to manage multiple tasks simultaneously.

Ability to work independently in the field while maintaining attention to detail. Proficiency with electronic documentation systems, particularly EVV timesheets. Problem-solving skills to address issues promptly during home visits or administrative processes. Compassionate attitude with a focus on patient-centered care.

Valid driver’s license and reliable transportation are preferred for field visits. This position is an integral part of our home care team, ensuring that patients receive compassionate support while maintaining efficient operational workflows.
