Care Coordinator: Court Supported Services Division (CSSD)- 7126

OBJECTIVE OF POSITION: Coordinate and monitor the implementation of plans that address the behavioral health needs of the client. To work in partnership with the client and treatment team to coordinate and triage daily tasks and care for clients. Communicate and function as a liaison between services and providers. Complete any necessary reporting in a timely manner.

Advocate and support clients to address treatment barriers such as access, transportation, etc. in order to increase the independence and self-efficacy for individuals and families within our services.
