LPN - Home Health

Works under the supervision of the Director of Home Care. #Directly supervised by the RN staff to provide nursing care according to established Home Health policies and procedures. #Provides skilled nursing care to clients in their place of residence, contributing to the planning, implementation, and evaluation of nursing care to meet the age specific need of clients.

#Assist with patient teaching, observation and delivery of nursing care. # # # Education, Work Experience and Licensure: ### #Graduate of an accredited school of practical nursing. ### #Holds a current nursing license in good standing for Ohio and West Virginia.# ### #Minimum of one year nursing experience required. ### #Six months to one year home health experience desired.

#Previous experience preferred. ### #Valid driver#s license, proof of auto liability insurance and clean driving record. #A clean driving record includes, but is not limited to the following: #no OVI, no reckless operation, and no failure of physical control on driving record. #If such violations should occur after employment, employee may not continue working in the Home Care department.

#Citations will be reviewed on a case by case basis. ### #No criminal record as validated by criminal background check, including fingerprinting.
