- Job Overview. Schedule: Monday-Friday (8AM-5PM). As an In-Clinic Health Navigator, you will build meaningful relationships with community members and Legacy patients while helping them access medical care, stay connected to treatment, and adhere to medications for HIV prevention or treatment through Legacy Pharmacy partnerships. Your compassion, communication, and cultural awareness will help patients move confidently through the continuum of care and make informed decisions about their health.You will serve in both clinic and community settings, connecting individuals who need prevention services with Legacy care. As an integrated member of the clinical care team, you will bring health education and practical navigation support to conversations about HIV/STI testing, Pre-Exposure Prophylaxis (PrEP), Post-Exposure Prophylaxis (nPEP), HIV Treatment as Prevention (TasP), medication adherence, and safer sex practices.This is an opportunity to join a collaborative healthcare family where your passion for patient support can make a lasting difference in community health. Implement health education programming in one-on-one and group settings to increase patients’ knowledge to prevent illness, manage health conditions, and improve overall wellness.Use health education, navigation, disease self-management, goal setting, motivational interviewing, and other proven techniques to meet specific needs and assist patients in setting health-related goals.Provide information, referrals, and assistance with linkage to services to improve patients’ independence and overall health while supporting their varying routes throughout Legacy.Provide point-of-care testing for HIV and other health conditions in a clinical setting.Contribute to the development of new health education programming identified through interactions with individual, clinic, and community participants.Serve as a member of an interdisciplinary clinic and care team.Promote effective working relationships with internal and external stakeholders.Conduct brief assessments of patient behavioral, social, and medical needs, readiness for health behavior change, and progress over time.Establish and maintain referral processes, resource lists, and other community networks to effectively link patients to needed health information and services.Work within the context of a primary care medical home, applying a team-based approach to care while partnering with patients and families to promote timely access to care, culturally sensitive and language-appropriate understanding of care, continuity of care, and whole-person health through appropriate self-management skills.Meet a high standard of customer care to ensure a positive patient experience.Document all patient services in the electronic health record and other data systems as required.Manage patient schedules and desktop in the electronic health record system.Meet assigned performance metrics, including those related to grant or contract requirements, if applicable.Perform other duties as assigned.Minimum Qualifications. High School Diploma or equivalent. Required Years of Experience3 - 5 years of relevant experience. Desirable Education and/or Experience. Bachelor’s Degree in a related field with a minimum of 1 year of relevant experience preferred. Experience performing point-of-care testing to clients in a clinical or community setting. Demonstrated actions and activities with respect to HIV care and prevention. Provides examples of performing education to clients of diverse cultures and backgrounds in one-on-one and/or group settings.Previous experience with Electronic Health Records Systems (EHRS). Ability to demonstrate core competencies of HIV and HIV Prevention within 3 months of hire.Required Knowledge, Skills, and/or Abilities. Excellent communication skills and customer service skills.Adept in decision making responsibilities.Working knowledge and understanding of statistics and graphical representation principles.Strong computer skills (MS Office). Demonstrated competency with an EHRS system. Ability to manage multiple deadlines and prioritize tasks effectively.This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management. Ability to demonstrate core competencies of role within 90 days of hire. Desirable Knowledge, Skills, and/or Abilities. Bilingual English/Spanish fluency.About Legacy Community Health.
