Developmental Services Consulting Registered Nurse -Monday-Friday, Daytime

The Developmental Services Consulting Registered Nurse (RN) is responsible for the management and oversight of individual’s health care needs. The DS Consulting RN advocates for health supports that reflect the individual’s goals, preferences, and personal history in collaboration with individuals, families, guardians, case managers and the overall Team.

The DS Consulting RN supports team planning, incident response, and transitions in care, in addition to promoting preventative health practices, managing chronic conditions, and facilitating access to primary and specialty care. The RN provides training and ensures all delegated tasks comply with the Vermont Board of Nursing standards, ensuring accountability and clinical integrity, as well as adherence to CCN policy and protocol.
