Case Manager II Community Health

FLSA:  Non-Exempt/ Non Union Job Mission:  Improve health outcomes for these with HIV by facilitating access to services through eligibility determination, resources navigation, and benefits assistance. Support the Health Integrated Services department by addressing barriers to services, connecting individuals to community resources that promote health, stability and self sufficiency.
