Assumes accountability for assessing, coordinating # implementing care # service from admission through discharge for assigned patients.#Coordinates communication of all patient related information to appropriate personnel internally # externally. Adheres to agency standards # ensures confidentiality of patient information # patient rights.

Requirements:# RN License in the State of Rhode Island or a state participating in the Nursing Compact BSN preferred. For AD graduates applying, must provide evidence of enrollment for BSN program with completion within 3 years Basic Life Support certification from the American Heart Association One year acute care nursing experience# One year home care experience Valid driver#s license # auto liability insurance as mandated by State of RI#
