Qualifications

2 years of experience in long term care, licensed social worker or bachelor’s degree preferred, must possess excellent communication and organizational skills Essential Job Functions: Oversee onboarding and training of all new admissions directors, discharge planners, and social workers. Develop one-to-one relationships with residents and families.

Conduct monthly meetings and trainings for admissions and social services Quality Assurance responsibilities include conducting mock surveys and regular auditing. Manage all referral source accounts.
