Communicates with the attending physicians, other hospice team members and other agencies as needed to coordinate optimal care and use of community resources for the patient/family. Document all services provided and submit on appropriate forms. Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.

Report changes in patient condition and needs to RN Great time management skills Qualifications: At least 1 year experience with either LTAC or Surgical unit Current Louisiana license Must be able to work flexible schedule including 12 hour shifts Have dependable transportation, driver�s license and car insurance Make a difference in the lives of others � and your own � with Sage Specialty Hospital.
