LVN Hospice Continuous Care Nurse

Charlin Hospice LLC is seeking a compassionate and dedicated LVN Hospice Continuous Care Nurse to join our team as a contractor. As a member of our team, you will have the opportunity to provide high-quality care to patients in the comfort of their own homes during their end-of-life journey. **Job Overview:**As an LVN Hospice Continuous Care Nurse, you will be responsible for providing continuous nursing care to hospice patients in their homes.

You will work closely with patients, their families, and a multidisciplinary team to ensure that the physical, emotional, and spiritual needs of the patients are met.

Your role

will be crucial in providing comfort and support to patients during this sensitive time.Responsibilities:- Provide skilled nursing care to hospice patients in their homes

• Monitor and assess patient's condition and report any changes to the interdisciplinary team
• Administer medications and treatments as prescribed
• Provide emotional support and education to patients and their families
• Work collaboratively with other healthcare professionals to ensure comprehensive care for patients
• Maintain accurate and up-to-date patient recordsRequirements:- Current LVN license in the state
• Minimum of 2 years of experience in hospice care
• Strong communication and interpersonal skills
• Ability to provide compassionate care to patients and their families
• Knowledge of end-of-life care practices
• Must be able to work independently and as part of a team
• CPR certification
