Care Coach - Cluster J (Bloomington, IN & surrounding area)

The Care Coach position has been created to lend support to caregivers that are caring for a family member with complicated needs. As a care coach, you will go to the homes of our caregivers and help them find resources, offer support, and build good will. The goal of a Care Coach is to make 1 visit per quarter and build support for the caregiver.

Encouraging self-care and helping the caregivers set goals for themselves.
