Job description

RESPONSIBLE FOR

  1. Leading in making decision quickly to patient care.
  2. Performing professional nursing care to patient.
  3. Direct supervision and co-ordination of the activities in the allocated unit / ward, in line with the Policies and Procedures of Nursing Services.
  4. Responsible to keep Unit Manager informed of any incidents or reportable situations that need nursing administrative involvement.
  5. Assisting in monitor lower categories staff and report to Unit Manager
  6. Involvement in Clinical Audit or Hospital Audit. JOB DESCRIPTION 1.0 ORGANIZATION AND MANAGEMENT
  7. Adherence to nursing care vision, mission, goals, and objectives, aligns with the scope of nursing services, and performs shift duties as assigned by the Unit Manager to ensure high-quality care rendered.
  8. Notifies the unit manager of any changes in the situation that could impact the standards of care.
  9. Obtains and handovers a report of patient progress, ensuring all doctor's orders are followed before changing or overtaking shifts and notifying any incidents or problems encountered.
  10. Visit patients in the unit with team members to assess conditions, re-plan nursing care, and determine nursing priorities based on patient needs and staff availability.
  11. Provides personalized, holistic patient care using the nursing process approach, maintaining accurate documentation in patient folders, and promptly reporting any abnormal findings to relevant doctors or consultants.
  12. Conducts nursing rounds on patients, providing emotional support and accompanying doctors while doing rounds, as well as assisting in procedures as needed.
  13. Executing the doctor's orders, coordinating with team members related to the patient’s treatment, and fostering effective communication with all disciplines, patients, and relatives to ensure patient satisfaction.
  14. Actively participates in the health education, dialogue sessions, and hospital activities.
  15. Provide clear, compassionate, and culturally sensitive communication with patients, ensuring their understanding of their condition, treatment options, and care plan comprehensively.
  16. Collaborate with the unit manager in administration, setting care standards, and fulfilling duties in the absence of the unit manager, while also carrying out assigned tasks and providing feedback.
  17. Charge accurately, incorporating charges for equipment and consumable items into the patient's charges and verifying their completeness.
  18. Inspect and document the supply of sterile packs to ensure the integrity of the packaging and maintain an adequate supply of unit stock from central sterile supply services.
  19. Acts on behalf of the Unit Manager in setting standards and expectations in the quality of care.
  20. Carries out duties and responsibilities of the Unit Manager in her absence as directed.

  22. Carries out assignments scheduled by Unit Manager and provide feedback 2.0 HUMAN RESOURCE AND DEVELOPMENT
  23. Accountable for personal and professional growth and keeps up to date with professional change by maintaining mandatory learning hours.
  24. Executes continuous professional development through annual competency evaluations and privileges every 3 years, and is proficient in performing calculations, chemo port needle insertion and removal, and medication calculations.
  25. Participates and contributes to the teaching program of the unit/ward and orientation of new members of staff to the ward routine.
  26. Engage in the supervision of staff, which includes: a. Assistant Nurses, Midwives, Care Assistants, and student nurses assigned to the unit for their clinical experience to achieve the clinical attachment objectives. b. Collaborates with the unit manager to evaluate staff performance, provide constructive feedback, and optimize staff, supply, and service utilization for efficient resource allocation and coordination with consultants for effective bed management.
  27. Participate and contribute to the development and maintenance of the nursing process, ensuring effective patient care and documentation.
  28. Participates in research initiatives and advocates for quality improvement through evidence-based practice. 3.0 POLICIES AND PROCEDURES
  29. Participates in the review and revision of nursing policies, procedures, and work instructions, providing valuable insights, promoting best practices for optimal patient care, and continuously improving the process.
  30. Inspect the Psychotropic drugs with the previous shift's nurse in charge and adhere to hospital policies and procedures in medication care, custody, and administration.
  31. Checks daily and audits resuscitation trolley periodically as scheduled.
  32. Promotes infection prevention and control by ensuring staff follow protocols, implementing surveillance activities, and demonstrating commitment through personal involvement.
  33. Maintain confidentiality of patient’s personal and medical history 4.0 FACILITIES AND EQUIPMENT
  34. Operates and maintains specialized equipment in a unit, guiding new staff on the use of it and maintaining accurate records.
  35. Collaborate with the unit manager to maintain equipment, medical supplies, consumable items, and drugs, ensuring proper functioning, service according to the planned preventive maintenance (PPM) schedule, and adequate stock and linen supply.
  36. Notify biomedical personnel of faulty equipment and assist in maintaining instruments and equipment through proper cleaning, handling, disinfection, and usage.
  37. Participate in a quarterly stock count with the finance staff. 5.0 SAFETY AND QUALITY IMPROVEMENT ACTIVITIES
  38. Participate in and support activities aimed at the continuous improvement of nursing services, including quality assurance, infection control, and fire and safety programs.
  39. Conduct thorough investigations, root cause analyses, appropriate response actions, and timely completion of incident reports within the stipulated timeframe.
  40. Collaborate with unit Manager to collect and evaluate clinical and administrative data, and participate in hospital-wide surveys like MSQH, JCI, IMS, etc. for continuous quality improvement efforts.
  41. Participate and contribute to the Baby Friendly Hospital Initiatives certification, with focus on promoting exclusive breastfeeding.
  42. Adhere to the Occupational Safety and Health Act and Regulations 1994, which mandate reasonable work care, employer cooperation, protective equipment use, patient safety goals, World Alliance recommendations, patient and family rights policies, standard people practice, and 5S activities. 6.0 ADDITIONAL DUTIES AND RESPONSIBILITIES IN THE SPECIFIC AREA IN ICU / HDU
  43. Trained and certified in BLS / ACLS / NRP.
  44. Demonstrate high standard of critical care in management of critically ill and injured patient with: mechanical ventilation, continuous renal replacement therapy (CRRT), pacemaker, acute myocardial infarction, etc.
  45. Records patient’s assessment according to the body system.
  46. Ensures continuous cardiac monitoring is maintained. Records rhythm strips every shift, pre, during and post completion of thrombolytic medicine or when arrhythmias occurs
  47. Provides health education and emotional support to patients and families.
  48. Interprets ECG and ABG, able to detect abnormality and inform consultants concerned.
  49. Ensures completeness of the 24 hours Critical Care Chart and other related forms.
  50. Liaises with consultants concerned to infuse inotropic support via a central venous line (CVL)
  51. Carries out pre-used test/check for equipment’s.
  52. Monitors patients using the care bundles where appropriate 7.0 DECISION MAKING Identify the extent and complexity of decision making required and the impact on the organization This post requires adequate level of knowledge in patient care, makes decision related to patient care and ensure compliance of Nursing policies and procedures. Decision may impact the quality of service to the patient 8.0 LATITUDE Identify the degree of choice or freedom in decision making Freedom of action is relatively high but constrained by the policies and procedures of the Nursing Services and Hospital. Issues of ethical and managerial significance affecting the Services and the Hospital must be reported to the UM. JOB REQUIREMENT 1.

Education

  • Registered with the Nursing Board of Malaysia with a valid license to practice as a Registered Nurse
  • Post Basic qualification in a related field
  1. Knowledge and Experiences:
  • Registered Nurse with at least 3 years of clinical experience and diversity in clinical experience.
  1. Skills & Competencies:
  • Possess certifications in BLS, ACLS, MTLS, PALS and NRP (according to the specialties)

  • Proficient in writing and conversing in English and Bahasa Malaysia.
