2026-2027 VOLUNTEER/MENTOR (Lunch Buddy, School Based Volunteers, and Mentors)

This application should be filled out for all applicants that desire to volunteer in the school setting

• Lunch Buddy, Mentor, Classroom, Athletics. ONLY COMPLETE THE ATTACHED MENTOR FORMS (Mentoring Form and Parent Mentoring Agreement Form) IF YOU DESIRE TO SERVE AS A BONA FIDE MENTOR . The completed forms should be submitted to the Parent Liaison at the school your child attends. Please allow at least 7 business days for processing.
