Position Summary

We are seeking a compassionate, skilled Registered Nurse (RN) to join our Home Care Team on a part-time basis during weekend day shifts. In this role, you will deliver high-quality, continuous nursing care and symptom management to patients living in their home environment, ensuring comfort and support for both patients and their loved ones.

Key Responsibilities

Conduct skilled nursing assessments, administer medications, manage symptoms, and perform hospice-specific interventions according to individualized care plans. Patient & Family Support: Provide end-of-life education, physical comfort care, and emotional guidance to patients and family caregivers. Interdisciplinary Collaboration: Work closely with physicians, medical directors, social workers, chaplains, and hospice aides to coordinate seamless, comprehensive care.

Documentation: Maintain accurate, timely, and complete clinical documentation in compliance with Medicare/Medicaid regulations and organizational standards. Travel: Visit patients across the local service area within scheduled weekend hours. Mesilla Valley Hospice is an Equal Opportunity Employer.
