FTE 1.0 ENVIRONMENT/PHYSICAL DEMANDS Standard Office / Sit, stand, walk, lift 15 lbs JOB DESCRIPTION POSITION SUMMARY: The Clinical Care Coordinator plays a critical role in ensuring seamless, coordinated care for clients admitted to the inpatient and residential SUD treatment programs.
This position provides clinical oversight of treatment planning, case management, assessments as needed, and discharge coordination. Working closely with a multidisciplinary team—including counselors, medical providers, case managers, and external agencies—the Care Coordinator supports client stabilization, engagement, and successful transition to the next level of care.
DUTIES AND RESPONSIBILITIES
Responsible for coordinating and communicating with managed care organizations, DBH, and other payors to ensure clinical services are provided. Maintain the confidentiality of patient information in accordance with the Prestige policies, the standards of the American Psychological Association, American Counseling Association, National Association of Social Workers and Department of Behavioral Health.
Assists with monitoring mental health service staff to ensure compliance with the health care policies and procedures, the policies and regulations of the facility; the responsibilities/duties of their specific position descriptions and the employee policies and procedures of the facility Ensure patients’ clinical documentation and department files are maintained appropriately On-site coordination of psychiatric services by provision of administrative support and patient monitoring Communicate concerns to the Clinical Supervisor/ Director and/or his or her designee in a timely manner with concerns which may have a significant impact on an individual consumer or on the mental health service program Monitors and Tracks all Treatment Plans to ensure he/she can provide timely updates under the Supervision of Clinical Supervisor.
Monitor Status Reports from various systems and EHRs, Credible i.e. Local Dollar & Medicaid Ensures all Diagnostic Assessments under the supervision of Clinical Supervisor/ Director remain up to date To maintain current and active utilization case load spreadsheet of all residential clinical staff on a daily basis Works closely with Billing team to ensure that all documents are completed in efforts to ensure authorization of services Keep compliance with employee standards of Prestige: this includes, but is not limited to, the maintenance of a working environment and personal appearance consistent with professional responsibilities, development of harmonious working relationships, and timely notification of supervisory personnel of absences from clinic.
Must be able to obtain and maintain appropriate licenses in accordance with the rules and regulations of the Department of Behavioral Health in Washington, DC Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.
Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations Develop and accurately maintain scheduling and event coordination Develop and adjusts coordination structure to ensure fit with operational realities Assist the clinical staff with recertification or quarterly report activities and care coordination efforts as needed and appropriate Assist in efforts to track and monitor progress by adhering to DHCF regulations pertaining to Prior Authorizations, proper documentation, proper discharge, etc.
Help facilitate and participate in all meetings and in-service trainings scheduled Provide training and orienting new clinical staff as needed Perform other duties as assigned EXPERIENCE AND QUALIFICATIONS Independent Clinical level licensure: LGSW or LGPC or LPC or LICSW , with approved supervisor certification (if applicable) Master’s Degree in Clinical Social Work, Psychology, Counseling, or related field 3-5 years experience is in a community-based behavioral health setting is required Knowledge of the Consumer Culture Current CPR certification Experience with Electronic Medical Record Systems such as Credible, GAINS, LOCUS and/or ICAM’s highly preferred Experience working with DC Government community resources, agencies and/or social services Working knowledge of medical terminology, HIPAA and 42CFR regulations, ICD and CPT coding highly preferred Proficient computer skills in Microsoft Outlook and other standard software applications.
