Coverage includes: Jackson Michigan Center Grass Lake Brooklyn Napoleon Chelsea Ann Arbor Ypsilanti Saline Manchester Spring Arbor Parma Concord (as needed) This role is ideal for an RN who values autonomy, strong clinical judgment, and the opportunity to grow alongside an expanding patient census. In This Role, You Will Conduct comprehensive nursing assessments Develop individualized plans of care Deliver skilled nursing interventions in the home Educate patients and families on condition management and medications Coordinate with physicians, therapists, and interdisciplinary team members Complete accurate and timely documentation in Well.
