Coordinator Patient Care - Full Time

The Patient Care Coordinator supports the organization’s population health and value-based care initiatives by proactively identifying and closing gaps in patient care. This position is responsible for coordinating patient outreach, scheduling Medicare Annual Wellness Visits and other preventive services, supporting chronic care management initiatives, and coordinating wellness and population health projects.

This is a remote work from home position that will require attendance to meetings on campus as scheduled.
