Job description

The Community Tele. PrEP Navigator serves as a trusted community-based resource supporting individuals in accessing HIV prevention services through the Philadelphia Tele. PrEP Program. The Navigator provides PrEP education, community outreach, telehealth navigation, client follow-up, and connection to supportive services.

The position will also support implementation research activities for the STEP-UP (Streamlining Telehealth for Equitable PrEP Utilization through Community Partnerships) study. The Navigator will work closely with organizational staff, healthcare providers, the Philadelphia Tele. PrEP Program, and the STEP-UP research team to reduce barriers to PrEP access and strengthen community-based HIV prevention.

MAJOR RESPONSIBILITIES: Community Outreach & PrEP Education Conduct community outreach and promote awareness of PrEP and Tele. PrEP services. Provide culturally responsive PrEP education and answer questions about accessing PrEP. Explain the Philadelphia Tele. PrEP Program and available COLOURS support. Engage clients in a welcoming, confidential, and nonjudgmental manner.

Tele. PrEP Navigation Assist clients with scheduling Tele. PrEP appointments. Help clients access technology and private space for telehealth appointments when needed. Assist clients with navigating laboratory testing through Labcorp/Quest, or applicable self-sampling options. Identify barriers to accessing PrEP and connect clients with appropriate supportive resources.

Follow up with clients after Tele. PrEP appointments and assist with next steps. STEP-UP Research Support Invite eligible clients to participate in required surveys and provide survey invitation information. Complete Outreach, Navigation, and Time-Activity Cost Logs accurately and on time. Maintain organized program and research documentation while protecting client confidentiality.

Communicate implementation questions or challenges to the designated research team. Supervision & Collaboration Report to the designated COLOURS supervisor and participate in regular check-ins. Communicate with the Philadelphia Tele. PrEP Program and STEP-UP research team as needed. Participate in required training, meetings, outreach events, and program activities.

QUALIFICATIONS

Experience in community outreach, health education, HIV prevention, healthcare navigation, or a related field preferred. Preferred candidate – High School Diploma or GED; Clinical experience with HIV prevention and harm reduction strategies; Demonstrated excellent oral and written communication skills; Comfortable using computers, telehealth platforms, email, and electronic documentation systems.

Demonstrates understanding of the cultural and ethnic diversity of sexual and gender minority people of color; Ability to multi-task; Willingness to travel to partnering agencies, client appointments, boards and committees that support program outcomes Ability to maintain confidentiality and accurate records. Reliable, organized, self-directed, and able to work effectively as part of a team.

Lived experience and/or strong knowledge of Philadelphia communities disproportionately affected by HIV is highly valued. Willing to work evening weekends, Flexibility is a must REPORTING RELATIONSHIP: Community Engagement Manager
