Our providers manage a defined patient panel with an emphasis on continuity, preventive care, and chronic disease management — a model built around lower daily patient volume and longer visit times than traditional fee-for-service primary care.

Each clinic is built around smaller patient panels, longer visits, and a care team (Medical Director, PCP/APP, nursing, and Medical Assistant staff) that works closely together, rather than the high-volume model of traditional fee-for-service practices.
