Position Overview

We are looking for a dedicated, organized, and tech-savvy Care Manager to join our remote team. This position requires a deep commitment to community service and holistic wellness. While the role is hybrid, candidates must reside in the servicing county and be available for local travel and driving for client visits, assessments, or community events.

Responsibilities

Conduct virtual or in-person assessments to identify client needs and goals Develop individualized care plans and provide ongoing case management Coordinate services across housing, healthcare, employment, and wellness resources
