Care Coordinator - Licking

BHP is currently searching for experienced professionals to provide full time case management services. This position requires experience in Community Support outreach activities including case coordination and conducting home and community visits. The ability to collaborate and communicate with other service providers is essential.

Must be able to engage and maintain professional client relationships. Responsible for setting and enforcing appropriate boundaries with clients. Associate Degree in Human Service field is required; a Bachelor’s Degree is preferred. Licensure is preferred (SWA, LSW, LPC). This position provides transportation to clients therefore candidates must be able to provide reliable transportation; valid Ohio driver’s license and proof of auto insurance are required
