Physical Therapist (PT) - 1099 Always Compassionate Health Schedule: Flexible Per Diem | Immediate Opportunities Location: Nassau County, Long Island, NY Employment Type: 1099 Independent Contractor Always Compassionate Health is seeking compassionate, reliable, and dedicated Physical Therapists (PTs) to provide high-quality rehabilitation services to patients in their homes.

Key Responsibilities

Conduct physical therapy evaluations to assess mobility, strength, balance, and functional limitations. Develop and implement individualized treatment plans based on patients’ needs and goals. Provide therapeutic exercises, gait training, balance training, and mobility interventions. Monitor patient progress and adjust treatment plans as needed.

Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility. Maintain accurate and timely clinical documentation. Collaborate with physicians, caregivers, and other healthcare professionals to coordinate patient care.

Qualifications

Active New York State Physical Therapist license. Degree from an accredited Physical Therapy program. Strong clinical assessment and rehabilitation skills. Excellent communication and organizational skills. Previous physical therapy experience preferred. Ability to work independently and collaboratively.
