General Summary of Position Med. Star Health is looking for Clinical Social Worker to join our team at our Georgetown University Hospital! The incumbent in this position is a professional clinical social worker who provides direct services to the patients . These services are related to hospitalization dealing with illness and post-hospital planning.

The incumbent also provides social work consultation leadership and teaching functions for the patients and their families/significant others as well as for the community. Adheres to NASW Code of Ethics and exemplifies the qualities and values of the social work profession. These functions are performed in accordance with all applicable laws and regulations and Georgetown University Hospital's philosophy policies procedures and standards.

Star Health can be your next great career move! Primary Duties and Responsibilities Provides clinical social work services to identified patients and families.

This includes outpatient and inpatient services. Includes individual and group counseling regarding the impact of illness and hospitalization. Collaborates with other health care professionals as part of a multi-disciplinary team. Facilitates communication regarding relevant psychosocial issues of patients and families to health care team through recording in medical records (inpatient and outpatient) consultation and participation in relevant meetings.

Functions as a coordinating link between patient family hospital and community resources to assure continuity of care. Represents social work perspective and serves as department representative to designated Hospital and Lombardi Cancer Center committees and community organizations.

Qualifications

Master's degree of Social Work from an accredited training program. required 3-4 years A minimum of 2 years experience with acute and chronically ill patients. required Hospital setting preferable required Experience with oncology patients preferred Licensed Independent Clinical Social Worker
