Complex Support Coordinator

The Complex Support Coordinator (CSC) will provide coordination of the intensive supports needed for adults with developmental disabilities who have complex multisystem needs. These adults require a high level of active engagement and ongoing mentoring. The CSC provides a variety of supports including conducting assessments, coordinating the development of complex support plans, establishing linkages to services, and coordinating cross-sector support for case resolution with the goal of improving the client’s access to supports and services, using an evidence-based, person-centered approach.
