Covers license renewal, CPR, and CEU expenses per calendar year H1B Sponsorship / H1B Transfer Assistance available for eligible full time employees The Physical Therapist (PT) provides and directs appropriate aspects of physical therapy care, including patient evaluation, treatment planning, therapeutic intervention, goal setting, patient and family education, progress monitoring, and documentation. The PT develops individualized plans of care designed to improve mobility, strength, balance, functional independence, safety, and overall quality of life. Reporting: This position reports directly to the Facility Rehab Director. Why This Role Matters Evaluate patients according to physician orders, payer authorization, professional standards, and facility guidelines Develop individualized physical therapy plans of care based on evaluation findings, patient needs, and functional goals Provide appropriate physical therapy interventions designed to improve mobility, strength, balance, endurance, coordination, transfers, gait, and functional independence Establish measurable treatment goals focused on achieving optimal functional outcomes Monitor patient response to treatment and modify treatment plans and goals as clinically appropriate Assess functional mobility and identify appropriate therapeutic interventions, assistive devices, and safety strategies Screen individuals who may benefit from Physical Therapy services Provide patient, family, and caregiver education regarding treatment plans, progress, mobility, safety, and functional outcomes Communicate patient status, progress, and needs with patients, families, caregivers, physicians, nursing staff, and the interdisciplinary team Complete accurate and timely evaluations, treatment notes, progress reports, discharge summaries, and required documentation Enter patient and treatment information accurately into EMR and rehabilitation systems Direct and supervise Physical Therapist Assistants, rehabilitation aides, and students in accordance with applicable requirements Coordinate treatment activities and goals with other members of the rehabilitation team Participate in interdisciplinary collaboration and rehabilitation programs Make appropriate referrals to other members of the rehabilitation and healthcare team when indicated Use professional and clinical judgment to help ensure patient and staff safety Attend required meetings, in services, training sessions, and educational presentations Prepare and submit accurate patient charges and billing information Support quality assurance, department operations, and program development Maintain and enhance clinical and professional knowledge through continuing education and professional development Maintain valid licensure and required certifications Qualifications: What We’re Looking For Currently completing, completed, or recently graduated from an approved Physical Therapy program. Graduating students and new graduates are encouraged to apply Must have or be eligible to obtain the appropriate California Physical Therapist license or authorization required to practice prior to providing Physical Therapy services Experienced Physical Therapists must maintain a current, active, and unencumbered California Physical Therapist license as required for the position CPR certification required. Candidates who do not yet have CPR certification may obtain it prior to or during the onboarding process Must be in good standing with applicable regulatory agencies and licensing boards New graduates and experienced Physical Therapists are welcome to apply Clinical experience gained through internships, externships, clinical rotations, clinical placements, or prior employment is valued Skilled Nursing Facility experience is beneficial but not required Strong clinical foundation in Physical Therapy Knowledge of patient evaluation, treatment planning, therapeutic intervention, mobility, transfers, gait, balance, strengthening, and patient education Excellent verbal and written communication skills Compassionate and empathetic approach to patient care Strong documentation and organizational skills Proficiency with EMR and rehabilitation systems or willingness to learn Ability to work effectively as part of an interdisciplinary team Strong desire to learn, grow, and build or continue a career in skilled nursing rehabilitation Professional presentation and conduct Commitment to continuing education and professional development Understanding of patient rights, confidentiality, and HIPAA requirements Physical & Working Conditions Skilled Nursing Facility environment Ability to move intermittently throughout the workday Ability to sit, stand, and walk for extended periods Ability to bend, squat, push, stoop, reach, and pull as required for resident care Ability to assist with patient transfers, mobility, positioning, gait activities, and therapeutic exercises Ability to lift and move up to 100 pounds with assistance and appropriate equipment according to resident needs and facility safety procedures Potential exposure to bloodborne pathogens, chemicals, fumes, and odors Ability to manage the physical, mental, and emotional demands associated with skilled nursing care Ability to work effectively in a 24 hour skilled care environment
