$40.00 per hour LPN Visit Rates: Wound Care Visits – $80.00 per case Comprehensive Benefits Package: Health, Dental, and Vision Insurance Flexible Scheduling: Full-Time, Part-Time, and Per Diem Opportunities Professional Development: Ongoing Training and Continuing Education Supportive Team Environment: Clinical Support, Mentorship, and Career Growth Meaningful Work: Positively impact the lives of children, adults, and families in your community Key Responsibilities Provide skilled nursing care, including medication administration, wound care, catheter care, and IV therapy Conduct patient assessments and contribute to individualized care planning Monitor patient conditions and report changes appropriately Educate patients and family members regarding treatment plans and health management Maintain accurate and timely clinical documentation in compliance with agency and regulatory standards Ensure patient safety while upholding the highest standards of nursing practice Collaborate with interdisciplinary healthcare teams and community providers Qualifications Active New York State LPN License Minimum of 1–2 years of nursing experience preferred (Home Care, Pediatrics, or Geriatrics) Strong clinical judgment and ability to work independently Current CPR/BLS Certification Experience with Electronic Health Records (EHR) preferred Excellent communication, organizational, and interpersonal skills Reliable transportation and/or valid driver's license preferred Apply Today Ready to make a meaningful impact while building a rewarding nursing career?
