$250k to $275k
Against the Jacksonville typical range
Job description
$250K–$275K base + monthly bonus — most physicians in this model earn $300K+. Monday–Friday, 8–5. No nights, no weekends, no hospital rounding. A physician-owned, multispecialty group in Northeast Florida is adding a dedicated Skilled Nursing Facility Physician to lead post-acute care across its SNF partner facilities.
This is a facility-based role only: you'll work inside the skilled nursing facilities, not in a clinic, and you'll supervise and collaborate with an established APRN team that handles day-to-day rounding. If you've been looking for geriatric/post-acute medicine without the hospital grind — predictable hours, a supportive team, and compensation that rewards you for the volume you manage — this is it.
What you'll do
Serve as attending physician for patients across the group's SNF partner facilities Supervise, collaborate with, and sign off for the APRN team covering those facilities Manage admissions, regulatory visits, care plans, and transitions of care (hospital → SNF → home) Partner with facility medical directors, DONs, and the group's primary care network for continuity Document in Athena (group-wide EMR) The schedule Monday–Friday, 8:00 am – 5:00 pm No nights, no weekends After-hours calls handled by a dedicated ED/after-hours physician service Compensation $250,000 – $275,000 base (depending on experience) Monthly productivity bonus — total compensation above $300,000 is typical Benefits Medical, dental, and vision (self-insured group plan) 401(k) with 3% employer match 3 weeks PTO + 7 paid holidays 5 CME days + $2,500 CME allowance annually Licensing and DEA fees covered Malpractice coverage Employer-paid life insurance, short-term and long-term disability Who we're looking for MD or DO, Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine Active Florida license (or eligible) and DEA SNF, post-acute, or long-term care experience preferred; physicians completing residency/fellowship in 2027 are encouraged to apply Comfortable supervising advanced practice providers and leading an interdisciplinary team Collaborative, organized, and committed to geriatric and post-acute care About the group Physician-owned and physician-led, with a large, established multispecialty network across Northeast Florida — you'll have primary care, specialty, and administrative support behind you Stable, growing organization with long-standing relationships with every major health system in the market Jacksonville offers no state income tax, beaches, a major airport, and a lower cost of living than other areas in Florida #MASC105
