Role Summary

As a PRN RN supporting WEST VALLEY patients, you’ll deliver skilled in-home nursing that promotes safe recovery, comfort, and independence. This role exists to ensure timely clinical oversight, clear communication, and accurate documentation across a fully remote workflow.

Key responsibilities include

completing comprehensive assessments, coordinating with physicians and the interdisciplinary team to implement and adjust plans of care, monitoring for changes in condition and escalating appropriately, and providing patient/caregiver teaching on medications, symptom control, and home safety. Precise, compliant charting is required using the HOMECARE HOMBASE EMR SYSTEM.

Required qualifications/skills:

• Active Arizona RN license in good standing
• Home health or comparable clinical experience
• Strong assessment, care-planning, and education skills
• Reliable time management and documentation in a fully remote workflow
• Experience with the HOMECARE HOMBASE EMR SYSTEM

How Your Day Flows

You begin by reviewing assigned visits and recent notes, then plan an efficient route across the WEST VALLEY. In each home, you focus on patient comfort, questions, and progress, while keeping communication steady with the care team as needs evolve. Between visits, you complete same-day documentation in a fully remote setup using the HOMECARE HOMBASE EMR SYSTEM, confirm upcoming appointments, and align any follow-ups so care remains seamless. On PRN days, you help maintain dependable coverage when schedules shift or weekend support is needed.

Mileage Support

Mileage reimbursement for eligible travel.

This role requires you pass the OIG exclusion list, MVR and have an Arizona fingerprint clearance card.
