PALLIATIVE CARE RN Part Time JOB SUMMARY: Reviews referrals Provides expert, individualized coordination of care for patients and families. Completes a comprehensive physical and psychosocial assessment of the patient and family to identify needs and areas for intervention. Ensures appropriate referrals to specialists when indicated and supports adherence to a treatment plan, including physician visits for evaluation and ongoing management.

Assists patients in accessing appropriate services and resources in the community to improve their overall health and well-being. Assist Nurse Practitioners with day-to-day program tasks and tracking palliative referrals. Alleviate pain, fatigue, nausea, and other distressing symptoms. Accurately document patient assessments, interventions, responses, needs, goals, and teaching in the EHR.

JOB REQUIREMENTS: Requires current RN licensure within state of branch location and three years of clinical nursing experience, preferably in a Home Care, Hospice or Skilled Nursing facility setting. Successfully completed nursing education from an approved school of nursing. Must be able to independently organize work, communicate patient needs, and provide patient education and coordinate patient visit schedule(s).

Must accept travel and working conditions as assigned. Must possess effective communication skills (oral and written), as well as, CPR certification.
