Intake Coordinator - South Gates, NY - Full-Time, Days

Join Lifetime Assistance as an Intake Coordinator and help individuals and families access residential and other agency services with clarity and compassion. You’ll coordinate referrals, applications, intake meetings, documentation, and service timelines while partnering with OPWDD, Care Coordination Organizations, community providers, and internal teams.

A related bachelor’s degree and two years of experience are preferred; candidates without a degree may qualify with four to five years of directly related experience.
