Case Manager II - Housing Navigation Center

The Housing Navigation Center Case Manager II provides ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual client needs. Responsible for conducting assessments for homeless clients and clients on the verge of homelessness to create a client centered individual service plan which identifies supportive services needed to transition to an independent life.

Additionally, responsible for the necessary follow up with local agencies to assure needed services are initiated and continue as planned.
