Physical Therapist- North County San Diego (Home Health)

Responsibilities

Evaluate and assess patients' physical conditions and develop individualized treatment plans. Implement therapeutic exercises and techniques to relieve pain and improve mobility, strength, and coordination. Educate patients and caregivers on exercises, safety precautions, and self-care strategies. Collaborate with an interdisciplinary team to provide holistic, patient-centered care.

Maintain accurate and timely documentation of patient progress and treatment plans.

Qualifications

Active Physical Therapist license in practicing state. Master's or Doctorate degree in Physical Therapy. Minimum of 1 year of experience as a Physical Therapist, home health experience strongly preferred. Current BLS, hands-on CPR certification. Valid driver's license and reliable, insured vehicle.
