40 Hours per week FT - Shift 8-4:30p
The Population Health Specialist partners with providers to improve patient outcomes through preventive care, chronic disease management, and care coordination. This role manages assigned patient populations, conducts outreach to engage patients and close care gaps, supports transitions of care, identifies and addresses barriers to treatment adherence, and collaborates with community resources and insurance payers to improve quality measures and value-based care performance. The position requires strong communication, organizational, and computer skills, with the ability to manage multiple priorities while delivering exceptional customer service in alignment with The Grand Experience values.
