/ Qualifications CPR/First Aid Certification Credential Copy (Or Activity Supervisor Clearance Certificate) Health (Sudden Cardian Arrest Certificate) Letter(s) of Recommendation (3) Other (Coaches General Training Program Certificate) Resume (Include experience in first aid, coaching techniques and rules and regulations.)
