What You'll Do
As a Behavioral Health Therapist III, you will provide comprehensive behavioral health services in a collaborative care environment where medical and behavioral health professionals work together to support the whole person.
Responsibilities include
Conducting behavioral health assessments and establishing diagnoses Developing individualized treatment plans with measurable goals Providing counseling and therapeutic interventions to children, adolescents, adults, and families Supporting patients in achieving self-management and wellness goals Responding to behavioral health crises and coordinating appropriate interventions Collaborating with primary care providers and interdisciplinary team members Assisting patients in managing chronic medical and behavioral health conditions Referring patients to specialized services when needed Providing education and support to patients, families, and staff Maintaining timely, accurate clinical documentation Ensuring compliance with confidentiality and regulatory requirements What We're Looking For We are looking for someone who combines strong clinical skills with genuine compassion and a commitment to serving others.
Required Qualifications Master's Degree in Social Work Current Kentucky Licensed Clinical Social Worker (LCSW, LMFT, LPCC) credential Minimum of two years of direct clinical experience Current CPR certification or ability to obtain and maintain certification Commitment to continuing education and professional development Preferred Qualifications Experience in integrated primary care or community health settings Experience working with underserved or diverse populations Knowledge of evidence-based behavioral health practices Bilingual English/Spanish skills are welcomed
