Physician - Family Medicine

• Salary Negotiable (Employees will receive 5% below the negotiated pay rate during their probationary status.) Responsible To : Medical Director Summary : Provide independent primary care in a variety of clinical settings to a specified population of non-emergency patients. Provide consultation and advice to others as a highly specialized practitioner. Continuation of this position is contingent upon funding allocations.

Minimum Qualifications

Medical Doctor Degree; Wisconsin’s Physician License; three (3) years experience as a PCP in a clinic or outpatient setting. State of Wisconsin and Federal DEA Certification. Medical Specialty License or Certification. Board certified or Board Eligible in Specified Area of Medical Specialty.

Must be credentialed. Items to be Submitted : Must provide a copy of diploma, license, degree, or certification upon employment. Employees are required to have proof of seasonal influenza vaccination; medical or religious exemption may be requested upon hire. Background information disclosure (BID) form Three (3) Current letters of references (personal, professional, and past employment).
