Licensed Practical Nurse: Pathways OTP Putnam- Per Diem- 6991

The Licensed Practical Nurse (LPN)/ Registered Nurse (RN) is a core member of the CHR OTP team, responsible for providing safe and effective Medication for Addiction Treatment (MAT) services to individuals with substance use or co-occurring disorders. The LPN/RN will support patients in community OTP clinics, through mobile van-based services, and within correctional facilities in collaboration with CHR staff, the Department of Correction (DOC) Safety and Security personnel, and DOC Health and Addiction Services staff.
