Job description
The Case Planner provides direct support to children, youth, and families in foster care, promoting safety, permanency, and well-being. This role involves developing and implementing service plans, conducting regular assessments, coordinating services, and maintaining compliance with agency and regulatory standards. The Case Planner works collaboratively with families, foster parents, and internal and external partners to ensure timely progress toward permanency goals.
