Case Planner (Brooklyn)

The Case Planner provides direct support to children, youth, and families in foster care, promoting safety, permanency, and well-being. This role involves developing and implementing service plans, conducting regular assessments, coordinating services, and maintaining compliance with agency and regulatory standards.
