Clinical Pharmacist, Inpatient Pharmacy

Job Summary

·         Reports to Manager, Pharmacy Services. 

·         Compounds and dispenses medication pursuant to a physician order.

·         Monitors drug therapy for appropriateness of drug, dose, route, and duration of therapy. 

·         Screens for drug interactions, incompatibilities and contraindications. 

·         Recognizes potential adverse drug effects and intervenes as appropriate. 

·         Provides drug information to physicians, nurses, allied health professionals, and patients. 

·         Participates in interdisciplinary team conferences and other patient-care activities. 

·         Cares for all age group patients (birth and above).
