Licensed Practical Nurse - Memory Support Unit PT

The Licensed Practical Nurse (LPN) provides direct nursing care, health monitoring, and clinical support to residents in Memory Support and Assisted Living communities. Working under the supervision of the RN Nurse Manager and in collaboration with healthcare providers and caregivers, the LPN ensures residents receive safe, compassionate, person-centered care that promotes dignity, independence, and quality of life.

The LPN is responsible for medication administration, resident assessments, care coordination, documentation, and supporting the overall health and well-being of residents, including those living with dementia and other cognitive impairments.
