Registered Nurse (RN) - Hospice & Home Health

Key Responsibilities

Conduct comprehensive head-to-toe patient assessments and build customized care plans tailored to individual goals. Perform wound care, IV therapy, symptom management, and medication reconciliation in the field. Serve as the primary clinical liaison, coordinating directly with physicians, families, and our interdisciplinary team.

Provide vital patient and caregiver education on disease progression and emotional guidance through complex care journeys. Utilize EMR systems to ensure timely, accurate charting that meets regulatory compliance.

Qualifications

Current Texas RN license and BLS/CPR certification. 1+ years of clinical nursing experience (Hospice preferred). Reliable vehicle, valid TX driver’s license, and auto insurance.
