Now Hiring: Clinical Quality & Care Coordinator

South Plains Rural Health Services (SPRHS) is seeking a Certified Medical Assistant to join our team as a Clinical Quality & Care Coordinator. This position helps patients navigate referrals, complete recommended screenings, and stay on track with chronic condition care.

The coordinator works closely with providers, nursing staff, and the Referral Department to support timely patient follow-up and improve quality of care. This is a nonsupervisory position.

What You'll Do

Coordinate outgoing specialist referrals and diagnostic imaging orders with providers and the Referral Department.

Follow up with patients about scheduling, next steps, and questions about accessing care.

Track outside records and results, document updates in the electronic medical record (EMR), and help ensure referrals are completed.

Partner with providers and nursing staff to support care coordination and patient follow-up.

Maintain patient registries for conditions such as diabetes and hypertension and contact patients who are overdue for visits or screenings.

Support population health initiatives, Uniform Data System (UDS) reporting, Patient-Centered Medical Home (PCMH) activities, and quality improvement projects.

Assist with Plan-Do-Study-Act (PDSA) activities to test and improve care processes.

Maintain accurate, confidential patient records in accordance with HIPAA and SPRHS policies.

What We're Looking For

Medical Assistant certification required.

At least one year of experience in a clinical setting; community health center experience preferred.

Working knowledge of EMR systems, care coordination, and chronic disease support.

Strong organization, communication, and follow-through skills.

Ability to work collaboratively and protect patient confidentiality.

Bilingual in English and Spanish preferred.
