Home Health RN

Overall responsibility for management, oversight, coordination and provision of care for a group of patients. Responsible for providing for the physical, mental and social well-being of patients using evidence-based practice. The role requires case management skills directed toward achieving positive health outcomes, coordination with an interdisciplinary team, assuring adequate financial coverage for the care of patients and appropriate utilization of resources.
