Job description
The Community Health Worker supports Medi-Cal–eligible adults involved in the justice system as part of California’s Medicaid 1115 JI Reentry Initiative. This role provides outreach, care navigation, and non-clinical case management services to individuals transitioning from incarceration in state prisons, county jails, back into the community.
Working with clients during incarceration, the Community Health Worker helps ensure continuity of Medi-Cal coverage and connection to essential health care services, medications, Enhanced Care Management (ECM), and community-based supportive resources. The position plays a key role in promoting successful reentry, improving health outcomes, and reducing recidivism through culturally responsive, client-centered support.
