Description ABOUT APM Asociación Puertorriqueños en Marcha (APM) is a nonprofit organization in Philadelphia, PA providing human services to the community including early childhood education, foster care & adoption, child welfare, mental & behavioral health, community and economic development, housing, violence intervention, and services to community schools.
We have been helping families achieve their greatest potential since 1970. Fee for service position with potential to become full time employee with benefits DUTIES AND RESPONSIBILITIES:
- Provide counseling services to members in Drug & Alcohol program
- Complete Intake, treatment plans, evaluations and discharges on time as prescribed in the policies
- Provide Individual and group therapies consistently and family therapy when needs
- Use ASAM evaluation at the beginning and at every treatment plan sessions
- Complete documentation on time as prescribed in the policies and procedure manual
- Ensure all members provide UDS every week without fail
- Apply for BHSI for clients who doesn’t have insurance
- Co-ordinate with Septa logistics care for bus passes for members to come to facility
- Co-ordinate with Psychiatrist for evaluations, suboxone prescription for members and case consultations
- Work closely with supervisor and complete required supervision hours and case consultations
- Ensure all documentation and data are complete and in compliance with APM, city, state, and federal regulations
- Makes referrals to appropriate levels of care based on ASAM criteria
- Maintains strong interagency community, multidisciplinary and government agencies relationships.
- Maintains ongoing communication with other community treatment facilities, primary care physicians, hospitals, schools, multidisciplinary units, and Probation/Parole officers, the Forensic Intensive Recovery (FIR) unit, etc. as necessary.
- Maintains a caseload that supervisor deemed appropriate in your full time position
- Participate in meetings with the agency interdisciplinary team as clinically necessary.
- Participate and maintain required CE credits and other mandatory trainings
- Ensure all charts are up to date
- Actively engage in program expansion and development
- Works closely with the Clinical Director and set quarterly targets and achieve them
- Must be able to provide training and supervision if more counselors are hired for DA program
- Provides updated FBI, PA criminal background, child abuse, CPSL and CPR clearances before the start date
- Performs other program related duties as assigned by the Clinical Director.

Passion to work with Drug & alcohol population Provide genuine care and support for members while maintaining respectful boundaries Self-motivated and take proactive steps to develop and expand DA program under the supervision of Director Ability to take initiative and collaborate with other resources, agencies, hospitals and judiciary system in the community Represent APM Drug & Alcohol program with pride and joy QUALIFICATIONS: Bachelor’s Degree in the human services or related field with a minimum of 1-year work experience with clients in a rehabilitation, social services, mental health or related services.
CAC, CDAC licensed or qualified Completed ASAM training Trained in Motivational Interviewing Bilingual (English/Spanish) Proficiency in the DAP documentation Proficiency in the use of Microsoft Word Experience in facilitating group therapy Working with medication managed populations Strong verbal and written communication skills APM is an equal opportunity employer.
