POSITION SUMMARY
The Program Specialist provides support for the nurse home visiting, case management, and care coordination services including Babies First, Ca. Coon, and Family Connects Oregon. Family Connects is an evidence-based universally offered nurse home visiting program that connects parents/caregivers of newborns to the community resources they need after birth.
The model was developed in 2008 through a partnership with Duke University, the Center for Child & Family Health (CCFH), and the Durham County Health Department.
