Case Facilitator I - Adult

An Adult Case Facilitator I – The Case Facilitator I will be a member of the De. Kalb Outpatient treatment team, primary job duties include providing case management and activities of daily living skills to adult consumers, client reassessment and completion of ANSA, recovery plans and reviews, referral and linkage to services, and client release.

Education

High school diploma or equivalent. Experience: Must meet OBHP requirements =Two years’ experience in mental health or community services preferred. Licenses: Must possess a valid stste issued Driver’s License Shift: Full-time, First shift, M-F, No holidays or weekends. Salary baed on expierience. Excellent benefits. EOE
